Work Permit Process

Steps to apply for a work permit:

1. Students can pick up an application in the counseling office or click here to download the printable document.
2. Once the application is completed, you can drop it off at the counseling office to Ms. Rodriguez. (incomplete forms will not be accepted)  
​3. The permit will be ready 2 days after you turn in the application. 
 

 Before you submit your work permit, please make sure: 
  • The student, employer, and parents should fill out the form. 
  • The student's social security number should be written on the form and should be verified by the parent.​
  • The student must print your full first name and last name as it appears on school records (If you have 2 last names, write down both last names on the application).
  • The student must write their Overfelt ID number at the top of the application.
  • The applications will not be accepted if not filled out completely.
  • Electronic signatures are NOT accepted.  Only wet (ink) signatures for minors, parents, and employers are accepted.
 
Important Information
  • If the student has more than one "F" in the most current grading period, the student needs to see Mr. Chiala.  Mr, Chiala will then determine if the application is approved. If Mr. Chiala approves the application, the student can turn in a signed application to Ms. Rodriguez in the counseling office.
  • The student has to re-apply for a work permit every grading period. (See the expiration date on your permit)
